The Docket Number field must be completed for each case. Do not leave this field blank.
The Defendant Name / Title of Action field must be completed for each case. Enter "Youthful Offender" or "Sealed" if appropriate.
The Statute and Section fields must be completed for each case. Do not leave blank.
Using "VTL 1180D" as an example, to correctly report statute and section: "VTL" should be entered in the Statute field and "1180D" should be entered in the Section Field.
When reporting a local ordinance, select the appropriate Ordinance Indicator from the dropdown list. If not reporting a violation of a local ordinance, leave the Ordinance Indicator field blank.
Check the Felony box only if reporting a felony offense. If not applicable, leave unchecked.
The Offense Description field must be completed for each case.
The Disposition Date and Disposition Description fields must be completed for each case.
When reporting an amended case, you must enter the original charge case detail first, then select "Amended" from the Disposition Description drop down list. Enter the amended case information in the Amendment Detail fields.
When reporting a fee, you must select the appropriate fee type from the Fee Type field dropdown.
When reporting a fee only, select "Fee" from the Disposition Description dropdown list. Do not select "Fine/Surcharge."
When not reporting a fee, leave the Fee Type field blank.
